INTRA-UTERO SURGERY


There is growing evidence that surgical operations performed intra-utero, give better results to fetuses affected with certain diseases. Surgeons who operate heart defects or blockages of the bladder, ensure that there are no surgical scars. In this regard, an article appeared in the latest issue of NEJM realize the success in cases of myelomeningocele (incomplete closing of the neural tube, protruded spinal cord, lower limb paralysis, increased cerebral spinal fluid, hindbrain herniation, and bladder and learning problems), performed in utero (Spina Bifida Study). Prenatal myelomeningocele repair reduces the need to implant shunts (40% in the prenatal group, 82% in the postnatal), improves the development and motor function at 30 months (40% of the prenatal group walks against only 22 % of the postnatal group).

The prenatal group has better mental development score. However, intra-uterine surgery is associated with certain risks to the mother: uterine dehiscence during labor that require cesarean sections, fetal breathing problems and preterm gestation (75% in the prenatal group, 15% in the postnatal group). In all cases intra-uterine surgery was performed before 26 weeks gestation. The study also found lower brain herniation after 12 months (4% in the prenatal group, 36% in the post natal). Surgical procedure to be improved through the development of better non invasive techniques: high-resolution real-time ultrasonography. If things go well, we are waiting for transplants of stem cells intra-utero and use of fetal bone marrow in fetuses with sickle cell anemia and immune disorders. Efforts are made to accurately identify fetuses at high risk to undergo surgery in utero.

CIRUGIA INTRA-UTERO

Existen evidencias crecientes de que las operaciones quirúrgicas realizadas intra-útero, condicionan mejores resultados en fetos afectos de ciertas enfermedades. Los cirujanos que operan defectos cardiacos o bloqueos de vejiga, aseguran que no quedan cicatrices operatorias. Al respecto, un artículo aparecido en el último número del NEJM dá cuenta del éxito obtenido en casos de mielomeningocele (conducto espinal no cerrado adecuadamente, médula espinal protruída, parálisis de miembros inferiores, incremento de líquido cefalorraquídeo cerebral, cerebro posterior herniado, problemas vesicales y del aprendizaje), realizados intra-útero (Spina Bifida Study). La reparación prenatal del mielomeningocele, reduce la necesidad de implantar shunts (40% en el grupo prenatal, 82% en el grupo postnatal), mejora el desarrollo y la función motora a los 30 meses (40% del grupo prenatal camina, contra solo 22 % del grupo postnatal).

El grupo prenatal tiene mejor score de desarrollo mental. No obstante, la cirugía intra-útero se asocia a ciertos riesgos maternos: dehiscencia uterina durante el parto, que requiere secciones cesáreas , problemas de respiración fetales y gestación pretérmino (75% en el grupo prenatal, 15% en el grupo postnatal). En todos los casos la cirugía intra-útero se realizó antes de las 26 semanas de gestación. El estudio comprobó asimismo menor herniación del cerebro posterior a los 12 meses (4% en el grupo prenatal, 36% en el grupo post natal). Estimados que esperan ser mejorados merced al desarrollo de mejores técnicas no invasivas : high-resolution real-time ultrasonography. Si las cosas van bien, se esperan trasplantes de células madre intra-útero y empleo de medula ósea fetal, en fetos con sickle cell anemia y desordenes inmunes. Se hacen esfuerzos por identificar en forma precisa a los fetos de alto riesgo a ser operados intra-útero.
