NEW CRITERIA FOR HUMAN DEATH

In 1968, the Ad Hoc Committee of the School of Medicine of Harvard University, established medical criteria for defining the state of human death: I) Absence of response and reception. No response to external or painful stimuli. II) Absence of spontaneous breathing, Apnea longer than 3 minutes. III) No reflexes, flat electroencephalogram (EEG), fixed and dilated pupils, lack of eye movement even with shock or twist or introduction of cold water in the ear, IV) absence of tendon reflexes. Criteria need to be reviewed immediately, according to new cell findings obtained at the Pasteur Institute (France). Experiments allowed to return to life to skeletal muscle and bone marrow stem cells (17 days post-mortem in humans and 14 days in mice). Findings reveal selective survival of certain body cells confronting maximum stresses.

According to Fabrice Chrétien (leading researcher), apparently dead stem cells from bone marrow and skeletal muscle regenerate large amounts of tissue. In this respect, the same scientist says that mentioned stem cells adopted a reversible state of hibernation with reduced metabolic activity, prolonged phase before the first cell division, high levels of reactive oxygen species and non committed transcriptional status. Paradoxically, hypoxia or anoxia was critical to maintain the viability and regenerative capacity of stem cells. Considering the above metabolic adjustments would not be wise to rely too much on flat EEG. We could be burying recoverable people. An EEG may not detect very weak brain electrical signals. In this regard, it has been suggested more specific tests to assess the vitality of brain : cerebral SPECT, brain panangiography and transcranial ultrasound. New methodologies are needed to establish criteria for cell death. The current definition of human death should be reevaluated. Perhaps the actual death definition only cover a state of reversible organic disabilities not able to support the overall operation of system functions.

NUEVOS CRITERIOS DE MUERTE HUMANA

En 1968, el Comité Ad Hoc de la Escuela de Medicina de la Universidad de Harvard, estableció criterios para definir medicamente el estado de muerte humana : I) Ausencia de respuesta y recepción. No respuesta a estímulos externos ni dolorosos. II) Ausencia de respiración espontánea, Apnea mayor de tres minutos). III) Ausencia de reflejos, electroencefalograma (EEG), plano, pupilas fijas y dilatadas Falta de movimiento ocular aún con golpe o giro o introducción de agua helada en el oído, IV) ausencia de reflejos osteotendinosos. Criterios que necesitan ser revisados de inmediato dado los nuevos hallazgos celulares obtenidos en el Instituto Pasteur (Francia). Los experimentos permitieron volver a la vida a células madre del musculo esquelético y de la medula ósea (17 dias post-mortem, en humanos y 14 dias en ratones). Hallazgos importantes porque revelarian supervivencia selectiva de ciertas células corporales, frente a estreses máximos.

Según Fabrice Chrétien las células madre de la medula ósea y del musculo esquelético regeneraron grandes cantidades de tejido. Al respecto, el mismo científico opina que las células madre mencionadas adoptaron un estado de hibernación reversible con actividad metabólica reducida, una fase prolongada antes de la primera división celular, niveles elevados de especies reactivas de oxigeno y un estado transcripcional alterado funcionalmente. Paradójicamente, la hipoxia o anoxia fué critica para mantener la viabilidad y capacidad regenerativa de las células madre. Considerando las adecuaciones metabólicas anteriores no sería racional confiar demasiado en los EEG planos. Podriamos estar enterrando a personas recuperables. Un EEG podría no detectar señales eléctricas cerebrales muy débiles. Al respecto, se han sugerido pruebas más específicas para evaluar la vitalidad cerebral : SPECT, Panangiografía cerebral y Ultrasonido transcraneal. Se necesitan nuevas metodologías para establecer criterios de muerte celular. La definición actual de muerte debería ser reevaluada. Tal vez la muerte actual sea un estado de Premuerte con incapacidad orgánica reversible para sostener el funcionamiento global de las funciones sistémicas.
